PLEASE COMPLETE THIS FORM IN ITS ENTIRETY. OUR TECHNICAL SUPPORT ENGINEERS WILL CONTACT YOU AND ATTEMPT TO RESOLVE ANY ISSUES PRIOR TO AN RMA NUMBER BEING ISSUED.

RMA Terms

• Under this Return Material Authorization (RMA), the returned product will be evaluated by CIS Secure technical support staff.
• In instances when a returned product meets certain criteria listed below, an evaluation fee will be charged according to its product category as follows: $200 Phones $600 Computers $600 Video Teleconferencing (VTC) $700 COMSEC $1000 Airborne
• If the returned product is found to be inoperable and under warranty, the returned product will be repaired and/or replaced and returned at no charge.
• If the returned product is found to be fully operational, an evaluation fee will be charged and it will be sent back to its place of origin.
• If the product is found to be inoperable and is not under warranty, an evaluation fee will be charged and a repair estimate will be provided.
• This RMA form must be signed and filled out in its entirety before any diagnostic procedures and repairs can be initiated on the returned product.
